Verizon - MetLife's MyBenefits Center 

   In addition to employees communicating with MetLife via phone calls,
   beginning on July 1, 2006, they can also access MetLife's MyBenefits to
   initiate and track their claim from their home computer and home email
   address.  To access the website:
   ·  Go to www.metlife.com/mybenefits
   ·  Enter company name (Verizon Communications)
   ·  On left side of screen click on "Register now." (This only applies if
   not registered. If already registered, enter user name and password on
   left hand side and click submit).
   ·  Once "Register now" is clicked, employee will be required to fill out
   first name, last name, SSN, DOB and email address. Additionally,
   employee will be able to create own user name and personal password.
   Once this information has been entered, click on submit and registration
   process is complete.
   ·  Please note, all information is submitted via a secure server, thus,
   no confidential information will be reviewed by unauthorized
   individuals.

   Once employees have logged on, they can:
   ·  Submit and track claims online
   ·  Receive email alerts to their home email account when a claim has
   been processed or the status changes
   ·  Download disability forms such as the Attending Physician Statement
   (APS) and Medical Authorization Form
   ·  Access frequently asked questions about the disability claim process

   In general, it should take less time to file a claim online compared to
   telephonically.  The reason why it should take less time is because many
   of the fields are able to be pre-populated based on the Verizon
   eligibility file sent to MetLife, thus, unless the employee needs to
   make any changes, many of the fields are completed for them.  In the
   telephonic process, the claim intake team does not have access to the
   eligibility file at the time the employee calls in their claim, and thus
   additional necessary questions are asked to initiate the claim.

   The employee is welcome to use whichever method of communication they
   prefer.
